ePACT Information
The Des Plaines Park District is proud to announce our adoption of ePACT Network-digital platform for collecting your child’s emergency and medical information-to improve efficiency and reduce our carbon footprint.
ePACT is a secure emergency network that collects medical and emergency contact information electronically. Not only will ePACT replace paper forms, but implementing the network ensures that our staff have a quick way to communicate with families in the event of an illness, injury, or other emergency.

Why are we using ePACT?
- To save you time and energy.
- No more long paper documentation for emergency information. With ePACT, you only need to complete your child’s information once, and then verify that it is still correct for additional programs or subsequent years. If you need to update information quickly, just log in and update it and we’ll be notified immediately.
- Reduction of paper waste
- Using Electronic forms mean less paper printed and photocopies between all of our camp sites. Families can always see what data the organization has by simply logging into ePACT.
- Better privacy and security
- ePACT’s high encryption levels and commitment to security means your data on ePACT is safe and secure. Digital access over paper forms allows you to track and update easily and securely.
How does it work?
- After your register for a program, you’ll receive an email invite to share information with your organization
- Click ‘Complete Request’ to create a free account, or log-in if you already have an existing ePACT account.
- Enter the required information, like medical conditions, and share it with your organization so that program staff have access.

- If a have a question regarding ePACT, who do I contact for support?
Answer: The Customer Success Team at ePACT is ready to support you! You can send questions and/or feedback directly to them via help@epactnetwork.com or you can call 1-855-773-7228 and choose extension 2 to speak with the team.
- I did not receive an email from ePACT, what do I do now?
Answer: If you are currently enrolled in a day camp, early education, or a before and after-school enrichment program and did not receive an invitation email, please

- Do I need to sign-up for ePACT for each program or year?
Answer: No If you’ve already signed up for ePACT through the Des Plaines Park District, you do not need create a new account. However, you are responsible for maintaining two things moving forward:
Your child’s current medical information. You will be prompted once a year to validate their existing information within ePACT; changes can also be made at any time during the year if information changes.
Your contact information. If this information changes at any time, you can update it in one place, and it will automatically be updated across all programs.
Each park district program may have specific waivers or forms to fill out each year, but the core information will auto-populate from program to program and year to year.
- I forgot my username or password, what do I do?
Answer: Your username is your email address. If you’re not sure, please contact ePACT at help@epactnetwork.com or 1 (855) 773-7228 and choose extension 2.To reset your password, please click ‘forgot password?” on the login page and follow the prompts.
- How do I log in to ePACT?
Answer: After you have created an account from the ePACT invitation email, you may access your account by logging in here. Please do not create a new account without first receiving the invitation email or your paperwork will not be linked properly to the Des Plaines Park District.
