Boat Rental Agreement

LAKE PARK MARINA BOAT RENTAL AGREEMENT and LIABILITY WAIVER

By making a payment to rent a paddle boat or kayak from the
Des Plaines Park District, you agree to the terms of the Rental Agreement and Waiver as shown,
and they are in force as if you had signed the forms.
If you do not agree to the terms set forth, please do not pay for a rental.
